OFP eligibility requires patients to be permanent residents of Ontario. Each patient is eligible for one funded IVF cycle per lifetime, with a potential additional cycle for gestational surrogates. The funded cycle covers monitoring, consultations, and baseline services, but does not include fertility medications or genetic testing.

One funded IVF cycle at Pollin Fertility includes six core stages:

1. Medical assessment, including consultations and counseling

Eligible applicants submit a $50 application fee, which is considered a donation to the foundation.
